Is it a one-tier cake? You could do one of those surprise-inside cakes with both pink and blue M&Ms inside, assuming the twins are opposite gender. If it's two tiers, you could tint the sponge of the one tier pink and the other blue.

There has been a lot of buzz about creating piñata cookies, I think this could be applied to cake as well.

Maybe question marks on the outside of the cake, and bake, a white cake with blue or pink food color that everyone will see once the cake is sliced.  Or you could even fill it with colored nonpareils, candy, chocolates etc for the big reveal. But given the moisture content of cake, my only concern would be the candies melting inside slightly, and not coming out so clean during the big reveal.

Laura Titchener did a great tutorial in CC Magazine V4I6 on How to make gender reveal piñata cookies for a baby shower.

@detty also put together a similar tutorial on CC

I immediatly tought about polka dots inside the cake. You can do 2 shades of blue or 2 shades of pink (if it's 2 boys or 2 girls) or blue and pink dots if twins are opposite gender.
