T-Shirt/scrub Cake - How Do I?

How do you make t-shirt/scrub shaped cakes? Anyone have a pattern/template I could use? Or a link to a good tutorial?
A friend wants me to make a cake similar to this for her graduation...

Wilton has a t-shirt shaped pan still available. Ebay is usually a cheap place to find one. OR, you could take a 1/2 sheet and carve one out. HTH
