My brother is getting married and I have been asked to make something similar to the above. (Hopefully you can see it!)
I'm not charging for it but am interested to know how much you think it would be worth and how many hours would it take you?
Details 4" fruit. Double fondant cover 7" carrot with cream cheese filling, crumb coat, fondant 9" chocolate Mudcake with chocolate ganache under fondant. 12" dummy tier covered in fondant

That's roughly 55 servings. I would generally say attach a per slice price to the cake, but I don't know your area (I start at $5 a serving)
This cake you are posting however has a bunch of extras that I wouldn't include in that price as I work from a CFL. Includes as follows...
- Ganache under fondant
- The double fondant you are saying for the top tier
- I know a lot of people add a charge for fresh fruit as a filling, and I most likely would as well because of the extra precautions and steps.
- Extra gumpaste flowers I generally price per flower. However depending on the design
(if it's just a few accent flowers) I try and roll it into the $5 price as much as possible.
-As a general rule of thumb, the extra dummy tier is priced almost the same as a typical cake of the same size due to the amount of time, and the materials it takes to decorate.
-Carrot and mud cake aren't cheap to produce.
I can barely see the cake, but to me it just looks like 4 tiers, covered in white, with a few basic punched flowers. As is in the picture (but using my recipes and cake flavors), I would charge the standard $5, and most likely add a few accent flowers for a color pop and still make a nice profit.
However with all the things you said that they wanted, I would charge $500+
AFor the equivalent of 112 servings (including the dummy cake) with tiered cake prices in the $5-7/serving range a good price range would be $560 - $784 plus setup and delivery.
