How Do You Get This Texture On A Cake?

My future aunt and uncle wants me to make this cake for their wedding. How do you get that kind of texture in the frosting?

It's "rustic" buttercream. You should experiment using the back of a spoon, a dinner knife blade, or a small spatula.
