I agree with everyone else about the delivery charge. I'm not in a rural area so it is different for me. BUT I am similar to you in that I just have a commercial kitchen-no walk-up store front, no sit-down place to eat. Since I bake everything made-to-order, I am doing cupcakes by the dozen and you get to chose two flavors. Many customers still only get one flavor per dozen, which is nice since most of my recipes can only be decreased so much to make 12 cupcakes.
I do a $10 delivery fee within my county for "normal" amounts of cupcakes (under 10 dozen). Anything more than that, I charge what I charge for cakes, $30. I have to pack my car differently, which takes time and I have to drive even more cautiously. Outside of my county, I do per order since I don't have too many outside my county.
