Does anyone know if home kitchens are allowed to be licensed in Blackstone, MA? Does it have to be a separate licensed home kitchen?

In MA, you do not need a separate kitchen. The best thing to do is call the Blackstone Board of Health and ask if home kitchens are permitted in your town. Then ask for the necessary paperwork as well as where to take a servsafe class. Then, call the Blackstone zoning board of appeals. The zoning board of appeals will send out a notice to your neighbors, look at your house plans, and then they will check for objections. If none,they will vote you in. Most likely, you won't have objections.
If you are on a septic system, they may not allow a home kitchen. Good luck!
