My sister had to give her kid up for adoption over 40 years ago. Her son has reached out to her asking if they could get to know each other. For his birthday she is planning a dinner and wants me to decorate a cake. Does anyone have any ideas. You would think he was mine. I want it to be special. The entire family is excited.

It's not very elegant, but maybe something like a puzzle piece, or a decoration on the cake that's missing and being put in, making it complete. Saying the family wasn't complete without him.......

And most people are just amazed when a nicely decorated cake is made for them, even if it's not personal.

Good luck! That is really really neat!
