I don't, sorry. But what kind of information would you need on it? I'm trying to think what it would look like. Would this be for decorated cookies or cookie jar cookies? Or both?

would love one as well. Do you have a copy of your cake order form? Just curious, how much do you charge for your cookies and what determines your cost?

thanks for the post.

Here is my cookie order form. It is in excel 2003. I would post as a pdf, but many of the features won't work that way.

All of the yellow boxes are drop down menus. I just colored them so you would know which. The date boxes are set to print "wednesday march 19, 2008". I found sometimes i wanted to quickly look and see the date and day.

The tax box, you need to put you tax formula in it. The total is set up to total the items, minus discount, plus tax, plus shipping. Because I realized i should not be chargeing tax on shipping (duh). also on the line items the qty is multiplied times the unit price. I figured out the discount manually because I could not make the formula work like I wanted.

The date at top you need to type in. I had it set to automatically update. But them everytime i opened it it changed the date.

If you look at the header and footer you will be able to change the logo, and contact information. *** If you do not see a header and footer it is because I could not upload the form in template form. So if you save it as a template on your pc, then you can add the header and stuff.

If you want to change the items int he drop down list : select the box (s) you want to change and choose the Data menu at the top, then validation. Then type what you want in the box.

And a hint, when you are changing things, do print preview to see the form with header and footer to make sure it fits on one page.

Good luck, Julia

Can you post it as a PDF document just so we can see it? I don't have Excel on this computer and can't even look at your cookie order form. Wah!!!

Thank you! I've been using Cake Boss for this, but I wanted something I could print out in case I ran into someone that wanted to order or for in-person events (like bridal shows, etc.).

i finally figured out how to save my print shop items in pdf files. the only problem is that it pushes everything to the edge. it's actually supposed to have 2 signatures at the bottom

The files are probably not available any more. This thread is 5 years old and a lot has happened to the CC site since then.
