It's included in my pricing. Because of rising gas prices, I've had to implement a delivery fee for anything outside of 30 minutes away (based on Yahoo! Maps mileage/time calculation).
i would include delivery and set up in wedding cake fees. but calculate distance and gas for delivery before you give the quote.
I charge a delivery fee over a certain mileage. If i have to head to the nearest city its $50 deliveryfee. Set up is included in the price of the cake. Local delivery is still free, but i might have to rethink that if gas prices don't come down.

My delivery fee is $20 for any location within a 10 mile radius, 50 cents per mile for every mile thereafter.
I don't because I do not want them picking it up themselves so it is included but I do charge extra for anything not local
If I were in the business I would definitely charge... at least $25. More if you have to drive more than 10 miles to the reception hall. Also, be sure to charge a cake cutting charge, if you are asked to perform that service.
