this is my first attempt at my order form. I will no doubt change things as time goes on and business develops. The form is for my own use hence I have not included any terms and conditions, although this is easy to add onto the bottom of Page 2 (it is designed to be printed double sided on A4).

I've since updated the form to include a section for allergies.
