I did this for a colleague whose anniversary is Valentine's day. They wanted their initials (S and M) incorporated in the topper. I made pink and white hearts out of royal icing to put on the topper and the sides of the cake. As you can see behind the cake, some of the hearts didn't make it to the cake :-(
